We understand that successful rehabilitation requires not only comprehensive services but also effective inclusion of both resident and family. In order to achieve this goal, our program is designed to include:
- Home Assessments before Discharge
- Family Care Plan Conference
- Referral for Home or Outpatient Services upon Discharge
